      <title>Avaliação e relevância do perfil de religiosidade/espiritualidade dos cuidadores de crianças e adolescentes com diabetes mellitus tipo I no enfrentamento da doença e sua relação com o controle metabólico</title>

Abstract: INTRODUCTION: Diabetes Mellitus (DM) is a chronic metabolic condition in which there is an increase in blood glucose levels due to failure in secretion or insulin action, or both. The discovery of Diabetes in a child raises doubts, uncertainties, revolt and despair in the family. The search for information is necessary to clarify the disease, in order to mitigate the strong impact that the diagnosis of DM causes and, the search for support in religion, through prayers and support in the Divine faith to lessen suffering. Caregivers report symptoms of depression, stress and anxiety, which may interfere with glycemic control of children and adolescents with DM1. Therefore, religiosity constitutes an important tool for understanding the situation, obtaining resources and strengthening to deal with the weaknesses imposed by DM. OBJECTIVE: The level of religiosity and spirituality was Verified, its use as a Coping strategy and, symptoms of depression, stress and anxiety in caregivers of children and adolescents with diabetes type1; and its interference in glycemic control. METHODS: Socio-economic and demographic data Were collected from caregivers of 59 children and adolescents with Diabetes and obtained dosages of Fasting Glucose (FG); Postprandial Glycemia (PPG); Fructosamine (FRUTO), HbA1c, calculated the glycemic variability-Δ HbA1c, total cholesterol (TC) and fractions: high-density lipoprotein (HDL-C); Low-density lipoprotein (LDL-C); Non-high-density lipoprotein (non-HDL-c) and triglycerides (TG). Levels of religiosity were obtained by the DUREL scale; the use of religious/spiritual coping was verified by the SRCOPE scale. The DASS21 scale analyzed symptoms of depression, anxiety and stress. Descriptive statistical analyses were performed. The Student's T-Test was used for the independent groups or ANOVA followed by the Tukey Test of multiple comparisons for 2 or more independent groups. Pearson's coefficient was used to verify the correlation between the variables. In all tests, a significance level of 5%&#xD;
was considered, and the data were analyzed with the aid of Statistica V10 and SPSS version 23 software. RESULTS: Among The 59 participants, 54.2% (n = 32) were less than 40 years old and the majority consisted of female caregivers 93.2% (n = 55). Regarding children and adolescents with diabetes 30 (50.8%) were male, 46 (78.0%) Older than 10 years and 38 (64.4%) Time of diagnosis exceeding 5 years. In assessing the religiosity of caregivers, high religiosity averages were observed in the three dimensions evaluated: ORA, NORA and IR. Regarding the use of religiosity in coping with DM1 by caregivers, scores of use considered high of the Total SRCOPE (3.7 ± 0.3) and the average use of the CRP score (3.3 ± 0.6) were observed. The NSRCOPE/PSRCOPE ratio was (0.5 ± 0.2), demonstrating that the use of PSRCOPE was 50% higher in relation to the use of NSRCOPE The severity of symptoms of depression, stress and anxiety was identified as severe in 22.0% (n = 13), 44.1% (n = 26) and 42.1% (n = 25) of caregivers respectively. Regarding glycemic control and lipid profile, it was observed that the mean GJ, GPP, HbA1c and Fructosamine were above the reference values for glycemic control goals for children and adolescents with DM1. The values of TC, HDL-C and LDL-C were in conformity with the referential values. There was an inverse and moderate association between Δ HbA1c and RI; Strong and inverse between FRUTO and ORA and, positive and moderate between TC and NORA. Positive and strong associations were observed between the factor N4 – Dissatisfaction with the other institutional - with anxiety and moderate with stress. Moderate and positive Correlations were identified between N1 – Negative Revaluation of God - with depression; Between N3-negative Revaluation of meaning - with depression; with stress and anxiety, and between N4 - Dissatisfaction with the other institutional - with depression. CONCLUSION: High levels of religiosity were observed in the three dimensions evaluated, as well as their use by caregivers of children and adolescents with T1D. It was also Observed that the use of negative religious-spiritual coping in coping with the situation is positively associated with the severity of symptoms of depression, stress and anxiety, implying the need to assess the emotional health of Caregivers, as well as offering support by health professionals to help relieve these symptoms.

      <title>Expressão de citocinas no estroma cervical de pacientes com neoplasia intra-epitelial cervical de alto grau após tratamento com Interferon α-2b intralesional</title>

Abstract: Introduction: Uterine cervix cancer is the second most common type of&#xD;
 cancer among women, being responsible for the death of, approximately 250 thousand&#xD;
 women a year, and its incidence twice higher in less developed countries. Nowadays, HPV&#xD;
 role is very well defined as a causative factor for the appearance of cervical cancer and its&#xD;
 precursor lesions, being the excised methods used a lot in the treatment of these lesions.&#xD;
 HPV infection is becoming more and more frequent in young patients. This fact expresses&#xD;
 the need for conservative therapies such as interferon, which does not alter the anatomy of&#xD;
 the cervix and preserves the reproductive future of the patients.&#xD;
 Objectives: Evaluate the expression of messenger RNA of the cytokines (IFN-&#xD;
  , TNF- , IL-2, IL-12, IL-4, IL-10, TGF- 1, TGF- 2 e TGF- 3) at the stoma of the uterine&#xD;
 cervix of patients with high level of CIN, before and after the treatment with IFN  -2b&#xD;
 intralesional and evaluate the presence of HPV of high risk in responsive patients and&#xD;
 therapeutical failure, before and after the treatment.&#xD;
 Casuistry and Methods: Study group was composed by 10 patients ranging&#xD;
 from 18 to 50 years with a diagnosis of high-grade cervical intraepithelial neoplasia who&#xD;
 did not underwent previous treatment. In this work it was used Intralesional Human&#xD;
 recombinant Interferon  -2b 3.000.000 U (Blauferon B R-Blausiegel). Applications were&#xD;
 done using a 1,0 ml syringe and 13 x 0,45 needle three times a week on alternate days&#xD;
 (Mondays, Wednesdays and Fridays), for 6 consecutive weeks, resulting in 18&#xD;
 applications. Biopsy collection was done before the first application and after the 18th one&#xD;
 of IFN  -2b. For analyzing the RNAm expression of the cytokines it was used the PCR&#xD;
 technique, using specific primers for each sequence of cytokines studied. After the PCR&#xD;
reactions, amplified products were submitted to polyacrylamide gel electro-1 phoresis at&#xD;
 10% and stained in silver. Evaluation of the viral load of HPV was done by hybrid capture.&#xD;
 Results: At initial diagnosis 60% (n=6) of the patients were CIN II and 40%&#xD;
 (n=4) of them were CIN III. Related to the treatment response: 60% (n=6) had an answer&#xD;
 while 40% (n=4) had therapeuthical failure. According to the habits and life conditions&#xD;
 questioned in the initial protocol (parity, smoking and number of sexual partners) its was&#xD;
 seen that 70% (n=7) were multiparas, 60% (n=6) were smokers and 50% (n=5) have&#xD;
 already had 3 or more sexual partners and the mean age of sexual intercourse was of 16,6&#xD;
 years (minimum of 14 and maximum of 18 years). About the expression of the cytokines&#xD;
 before and after the treatment with IFN  -2b we observed: lack of expression of cytokines&#xD;
 Th1 (IFN- , TNF- , IL-2 e IL-12), before the treatment with IFN  -2b in patients who&#xD;
 answered and also in those who failed the treatment. However, after the treatment with IFN&#xD;
  -2b, we observed that responsive patients expressed IFN-  , IL-2, TNF-  suggesting a&#xD;
 Th1 potential related to the healing. IL-10 expression (Th2) was absent in all patients,&#xD;
 before and after the treatment with IFN  -2b. IL-4 (Th2) was expressed before the&#xD;
 treatment with IFN  -2b, in 2 responsive patients and also in 2 patients who failed the&#xD;
 treatment. IL-4 expression, before the treatment with IFN  -2b, in patients with&#xD;
 therapeutical failure, suggests that the Th2 immune response might have contributed to the&#xD;
 persistence of the high level lesion. Yet, concomitant expression of IL-4 and TGF- 3 in&#xD;
 patient with therapeutical failure after the treatment with IFN  -2b, suggests that the&#xD;
 immune response of Th3 may have modulated the Th2 immune response, avoiding a&#xD;
 satisfactory answer to the treatment. Related to the Messenger RNA expression of&#xD;
 cytokines TGF- 1, TGF- 2 e TGF- 3, it was not possible to correlate the results with the&#xD;
 clinical response to the treatment with IFN  -2b, since one responsive patient presented&#xD;
 concomitant expression of TGF- 1 and TGF- 2 after the treatment with IFN  -2b, while&#xD;
other two patients who did not answer to the treatment, expressed 1 TGF- 2 and TGF- 3&#xD;
 after the treatment with IFN  -2b. We also observed that these patients with therapeutical&#xD;
 failure had concomitant expression of IL-12/TGF- 2 and IL-4/TGF- 3, suggesting that the&#xD;
 immune response of Th3 may had modulated the immune response of Th1 and Th2 during&#xD;
 the treatment with IFN and, because of that, there was a therapeutical failure. Controversial&#xD;
 results are found in works that analyze the presence of Th1, Th2 and Th3 cytokines in&#xD;
 premalignant cervical lesions. It was observed a significant fall of the viral load of high8&#xD;
risk HPV in responsive patients (p=0,0313, Wilcoxon’s test), after the treatment with IFN&#xD;
  -2b.&#xD;
 Conclusions: Treatment with Intralesional IFN  -2b in patients with High11&#xD;
level CIN had satisfactory clinical response in 60% of the patients. Th1 immune response&#xD;
 (IFN- , TNF- , IL-2) seems to be related to the decrease of the level of CIN, after the&#xD;
 treatment with IFN  -2b in responsive patients. TGF-  expression after treatment with IFN&#xD;
  -2b in patients with therapeutical failure suggests an immunomodulatory role of this&#xD;
 cytokine inhibiting the Th1 protector response. There was a significant decrease on the&#xD;
 HPV viral load of high risk in patients who answered the treatment with IFN  -2b.

Abstract: Introduction: Bone repair requires several treatment approaches including the use of material não absorvível s. Further understanding is needed in bone regeneration, fracture treatment and osteogenesis. Cells of the immune and bone system have a dynamic interaction in such a way that each system influences the path and functionality of each other. Materials and methods: Study approved by the Ethics Committee on Research and Animal Experimentation on 04/11/2014, opinion No. 304. Thirty-six male, isogenic mice, 2 months old, were used. Two surgical procedures were performed in the same animal: one with bone marrow ablation (GC-control group) and one with bone marrow ablation and implantation of non-absorbable material (GE-experimental group). Twenty-six mice were euthanized at 3, 7, 15 and 30 days for morphological study - neoformed bone volume and collagen fibers. Ten mice were euthanized at 15 and 30 days for RT-qPCR study. Results: The CG mice presented significantly higher bone volume (V%) than the experimental group (SG), mainly on the 150 postoperative day (DPO). The SG presented higher bone volume in the 300 DPO, with significant differences in relation to the third and seventh day. For the analysis of collagen fibers, the GE showed a higher percentage of collagen fibers in relation to the CG, mainly in the 70 DPO in relation to the 30. The GC showed a higher percentage in the 70 DPO but with a decrease in near-normal levels in the 300 DPO.O RT-qPCR showed higher TNFα expression for GE in 150,300 DPO and in GC in 300DPO. IL-10 showed higher expression in 150 DPO in GC and GE compared to GC and GE of 300 DPO. IL-33 showed higher mRNA expression in the GCs of 15 and 30 DPO relative to GE at the same time. INFɣ, IL-4 and IL-17A, had no expression for 15 and 30 DPO in GC and GE. Conclusion: There is a delay in EG bone formation, a higher percentage of collagen in this group and an increase in TNF-α expression caused by foreign body reaction.

Abstract: Focal segmental glomerulosclerosis (FSGS) and minimal change disease (MCD) are the main podocytopathies in childhood, clinically manifesting as nephrotic syndrome. The pathogenesis of these diseases has not yet been fully elucidated; however, the immune system is known to play a central role in the development and progression of glomerular damage. This study stands out as innovative by uniquely evaluating the presence of autophagy in podocytes and its association with the expression of CD36, TNF, IL-10, and inflammasome (NLRP3). Epidemiological data and renal biopsy fragments from 37 children, aged 3 to 18 years, diagnosed with FSGS (n=16) and MCD (n=21) at the Kidney Research Center (CePRim) of the Federal University of Triângulo Mineiro (UFTM) were analyzed. The expression of CD36, TNF, IL-10, and NLRP3 inflammasome was assessed by immunohistochemistry. The presence of autophagy and foot process effacement were evaluated using Transmission Electron Microscopy. Both groups presented proteinuria, hypercholesterolemia, and hypoalbuminemia, with no significant difference. A reduction in CD36 expression was observed in the MCD group compared to the control and FSGS groups, whereas NLRP3 expression was significantly higher in the FSGS group compared to the control. Supporting this finding, autophagic activity in FSGS was significantly higher compared to MCD. No significant differences were observed in TNF and IL-10 expression. The results indicate distinct inflammatory profiles between FSGS and MCD, with FSGS characterized by higher inflammatory activity mediated by the NLRP3 inflammasome, while MCD exhibits lower inflammation, as evidenced by the significant reduction in CD36 expression. These findings suggest CD36 as a potential biomarker for the differential diagnosis between FSGS and MCD and contribute to understanding specific inflammatory pathways, opening new perspectives for individualized therapeutic strategies.
