dc.description.resumoFeridas crônicas são complexas e envolvem uma avaliação individualizada. Para acompanhar o processo evolutivo destas feridas, os registros devem ser sistematizados, permitir uma visualização global, identificar riscos e maximizar a comunicação. Os objetivos deste estudo foram elaborar um instrumento de avaliação e evolução de feridas crônicas e verificar suas propriedades métricas. Trata-se de uma pesquisa metodológica, desenvolvida em três etapas: etapa I, elaboração do instrumento, denominado neste estudo, de instrumento de registro de feridas crônicas; etapa II, caracterizada pela validação aparente e de conteúdo; e etapa III, verificar a validade e confiabilidade do instrumento. Nesta fase, foi utilizada como estratégia a confiabilidade interavaliadores. Para testar a confiabilidade dos itens do instrumento, foram utilizados Kappa simples e ponderado dos escores gerais obtidos em relação às observações das pesquisadoras, o Coeficiente de Correlação Intraclasse (ICC), que foi de 0,914, e para verificar a validade de critério do instrumento, o coeficiente de correlação de Pearson, cujo resultado foi de 0,573. O instrumento foi aplicado em 71 pacientes com feridas crônicas, no período de outubro de 2014 a janeiro de 2015, nas unidades de internação de clínica médica e cirúrgica e no Ambulatório do Hospital de Clínicas da Universidade Federal do Triângulo Mineiro. Da amostra, 56,3% eram mulheres, 72% idosos, e 60,6% apresentavam doenças cardiovasculares. Quanto à etiologia das feridas avaliadas, a de maior porcentagem foram as úlceras por pressão, com 45,1%. A maior parte das concordâncias dos itens do instrumento foi classificada como moderada e substancial, sendo encontrado nos itens que descreviam o tipo de tecido necrótico (k = 0,754), tipo de exsudato (k = 0,630), e nos subitens dor (k = 0,683), odor (k = 0,800), edema (k = 0,613). Os resultados apontam que o instrumento deste estudo é válido e confiável. Sugere-se que o instrumento seja aplicado em estudos longitudinais e em populações maiores, com distribuição igual de úlceras de diferentes etiologias crônicas, a fim de contribuir para a melhoria da qualidade da assistência, com respaldo de documentação adequada, permitindo orientação para o tratamento e a continuidade do cuidado.por
dc.description.abstractChronic wounds are complex and involve an individualized assessment. To follow the evolution process of these wounds, the records should be systematized, providing a global view, identify risks and maximize communication. The objectives of this study were to develop an assessment tool and evolution of chronic wounds and verify its psychometric properties. It is a methodological research, developed in three stages: stage I, development of the instrument, called chronic wounds recording instrument; stage II, characterized by apparent and content validation; and stage III, to verify the validity and reliability of the instrument. At this stage, we used a strategy of inter-rater reliability. To test the reliability of the instrument the items were used simple weighted Kappa and the general scores on the observations of the researchers, the interclass correlation coefficient (ICC), which was 0.914, and to check the validity of the instrument discretion, the correlation coefficient of Pearson, with result of 0.573. The instrument was administered to 71 patients with chronic wounds, from October 2014 to January 2015, in inpatient units of internal medicine and surgery at the Clinic and the Clinical Hospital of the Federal University of Triângulo Mineiro. In the sample, 56.3% were women, 72% elderly, and 60.6% had cardiovascular disease. In the etiology of wounds evaluated the highest percentage were pressure ulcers, with 45.1%. Most of the agreements of the instrument items was classified as moderate and substantial, being found in items that described the necrotic tissue type (k = 0.754), type of exudate (k = 0.630), and pain in subparagraphs (k = 0.683 ), odor (k = 0.800), edema (k = 0.613). The results show that the instrument of this study is valid and reliable. The instrument is suggested that applied in longitudinal studies and in larger populations, with equal distribution of different etiologies chronic ulcers, in order to contribute to improving the quality of care, with proper documentation support, allowing orientation for the treatment and continuity of care.eng
